Private healthcare rebates available – please check with your healthcare provider as to your plan for dietetic services and / or diabetes education services to enable the “gap” cost to be determined.
* Concessional / Medicare - your GP can refer you under the chronic disease management (CDM) plan. This assists people living with a chronic and/or complex medical illness. Medicare rebates are available for treatment from allied health professionals including Dietitians and Diabetes Educators who are assisting in managing the illness. Your GP is required to prepare a management plan (called a GP Management Plan and Team Care Arrangements) so that your illness can be better managed with the assistance of allied health professionals.
The initiative allows a person with a complex and/or chronic illness to claim a Medicare rebate for up to five visits (in total) to certain allied health professionals within a calendar year. Chronic medical conditions that may be covered by the scheme must have been present, or are likely to be present, for six months or more. Discuss a referral with your GP, I can provide your GP with a referral form.
Example of costs for Dietetic or Diabetes Education under a CDM plan from you GP:
E.g: Initial consultation & follow up consultation $55 ($55 rebate from medicare) gap cost = $0. These appointments are fully bulk billed - please supply your current Medicare care for the billing.
**Travel / home-visit is subject to area & additional $10 - by appointment only
Please reschedule your appointment if required providing at least 24 hours notice. If you fail to attend your appointment - your consult will still be billed at the above costs.
For each appointment please bring a 3 day food & drink record / diary and any referral paperwork or pathology reports from your GP. These can be scanned and emailed in advance to: icarenutrition@outlook.com
